Pathogenesis of HIV infection: total CD4+ T-cell pool, immune activation, and inflammation.
نویسنده
چکیده
Recent studies have yielded important findings on the pathogenesis of HIV infection. HIV infection leads to immune dysfunction through CD4+ T-cell depletion (immunodeficiency) and immune activation (immunosuppression). In vivo imaging studies of nonhuman primates indicate that the total body pool of CD4+ T cells may provide more accurate quantitation of immune depletion in HIV infection than the peripheral blood CD4+ count. Immune activation appears to be driven by both a homeostatic response to CD4+ cell depletion and an inflammatory response to HIV infection. The evidence is mounting that ongoing inflammation and coagulation account for the increased risk of serious nonopportunistic events in patients with HIV infection. Studies in long-term nonprogressors indicate that the HIV-specific immune responses in these patients are distinguished by clonal expansions of antigen-specific CD8+ T cells. Additional study of the precise mechanisms that allow immunologic control of infection in these patients may contribute to development of vaccines and immune-based therapies. This article summarizes a presentation made by H. Clifford Lane, MD, at the International AIDS Society-USA continuing medical education program held in May 2009 in Chicago. CD4 and CD8 T cell immune activation during chronic HIV infection: roles of homeostasis, HIV, type I IFN, and IL-7.
Immune activation plays an important role in the pathogenesis of HIV disease. Although the causes are not fully understood, the forces that lead to immune dysfunction differ for CD4 and CD8 T cells. In this study, we report that the molecular pathways that drive immune activation during chronic HIV infection are influenced by differences in the homeostatic regulation of the CD4 and CD8 T cell p...
متن کاملHuman T Lymphotropic Virus Type I (HTLV-I) Oncogenesis: Molecular Aspects of Virus and Host Interactions in Pathogenesis of Adult T cell Leukemia/Lymphoma (ATL)
The study of tumor viruses paves the way for understanding the mechanisms of virus pathogenesis, including those involved in establishing infection and dissemination in the host tumor affecting immune-compromised patients. The processes ranging from viral infection to progressing malignancy are slow and usually insufficient for establishment of transformed cells that develop cancer in only ...
متن کاملAssessment of HTLV-I proviral load, HIV viral load and CD4 T cell count in infected subjects; with an emphasis on viral replication in co-infection
Objective(s): HTLV-I and HIV virus quantification is an important marker for assessment of virus activities. Since there is a direct relationship between the number of virus and disease progression, HTLV- I and HIV co-infection might have an influence on the development of viral associated diseases, thus, viral replication of these viruses and co-infection were evaluated. Materials and...
متن کاملImmune activation and inflammation in HIV-1 infection: causes and consequences.
Thorough research on HIV is progressively enabling us to understand the intricate mechanisms that link HIV-1 infection to the onset of immunodeficiency. The infection and depletion of CD4(+) T cells represent the most fundamental events in HIV-1 infection. However, in recent years, the role played by chronic immune activation and inflammation in HIV pathogenesis has become increasingly apparent...
متن کاملPathogenesis of HIV infection: implications for treatment and prevention
The immune systems of patients with HIV infection are characterized by an immunodeficiency that develops in the setting of a global immune activation and a complex array of HIV-specific immune responses. The immunodeficiency of HIV infection is the result of a decrease in both memory and naïve CD4+ T cells. Imaging studies of the total body CD4+ T cell pool reveal global depletion within all ly...
